How do I qualify
for the clinic?
Have two forms of
ID, proof of residency, proof of income and not have
health coverage. (see Who is Eligible for details)
What are clinic
hours?
The office is open
8:00 a.m. - 12:00 p.m. and 1 p.m. - 4 p.m., Monday and Thursday, 8:00 a.m. -
12:00 p.m., Tuesday, Wednesday and Friday.
Daytime clinic on
Monday and Thursdays is from 9 a.m. - 12 p.m. and 1 p.m.
- 4 p.m.
Door opens for
Clinic on Tuesday at 4:30 p.m.
How do I get a
medicine refilled?
If the physician
ordered refills, call our refill line at 775-1389, ext.
2 and follow the instructions. Refills must be
called in no less than seven days before you run out of
medicine.
How do I find
out about my lab work?
One of our medical
staff will either send you a card (lab results were
within acceptable limits) or call you (lab results were
not within limits).
When is the next
available appointment?
Call 775-1389, ext.
O, our receptionist will make you an appointment if you
have qualified as a patient.
